• Intellectual Disability: This typically involves having a full-scale IQ of 70 or below that occurred prior to the age of 22.

• Autism: A diagnosis based on diagnostic tools received prior to the age of 22.

• Developmental Disability: Individuals aged 0 through 8 who exhibit a high probability of having an intellectual disability or autism.

• Medically Complex Condition: Individuals aged 0 through 21 with a current medical evaluation from a licensed medical provider.

In addition to the diagnosis, individuals must also meet the following criteria:

• Demonstrated substantial adaptive skill deficits in three or more major life activities, such as self-care, language skills, learning, mobility, self-direction, and capacity for independent living.

• Qualification for an Intermediate Care Facility (ICF) level of care.

• Medicaid eligibility, as it is often required for accessing most ODP services.
