Position Overview: Lux Home Care Agency is seeking a qualified and experienced Support Coordinator to join our team. The Support Coordinator plays a critical role in coordinating services for our clients, ensuring their needs are met, and promoting their overall well-being. If you have a strong background in human services, experience in writing Individual Service Plan (ISP) applications, and a passion for making a positive impact in the lives of others, we encourage you to apply.

Key Responsibilities:

• Develop and modify Individual Service Plans (ISPs) for clients.
• Coordinate assigned services as part of an individual's treatment plan.
• Assist individuals in acquiring and improving daily living skills.
• Facilitate therapeutic activities and socialization opportunities for clients.
• Utilize the Department's data systems to maintain accurate records.
• Collaborate with a multidisciplinary team to support clients effectively.

Qualifications: To be considered for this position, candidates must meet one of the following qualification options:

Option 1:

• Bachelor's degree including or supplemented by at least 12 college-level credit hours in sociology, social welfare, psychology, gerontology, or another behavioral science.

Option 2:

• A combination of experience and training equivalent to 4 years of experience.
• Education including at least 12 semester hours of college-level courses in sociology, social work, social welfare, psychology, gerontology, or other social sciences.

Experience Requirements:

• 2 years of experience writing ISPs or coordinating services.
• Experience does not include hands-on personal care or maintenance of individuals' homes.

Additional Requirements:

• Excellent communication and interpersonal skills.
• Strong organizational and problem-solving abilities.
• Ability to work collaboratively in a team environment.
• A commitment to continuous learning and professional development.

Training Requirements:

• 40 hours of training within the first year of employment, including person-centered assessment, service plan development, data system utilization, communication skills, conflict resolution skills, documentation, and understanding the disabilities of participants served.
• 20 hours of annual training on the above topics.

Responsibilities:
